We would like to thank you for your interest in Dr. Cooper for your medical care. We strive to make your experience with us as pleasant as possible, and would welcome your comments or suggestions.
To schedule an appointment, you may call Dr. Cooper’s coordinator, Whitney, Monday through Friday 8:30 to 4:30 at +1 (214) 720-9357. You can e-mail her at weady@carrellclinic.com.
Because of a high volume of patients that request to see Dr. Cooper, we ask if you need to reschedule your appointment, that you give us 48 hours notice. Please be aware that if you do not come for your appointment at your scheduled time, you may be subject to a “no show” charge.
For those patients and athletes who are traveling from outside the Dallas-Ft. Worth area we have the following suggestions: A map link to the Carrell Clinic Center is available here. Our office is within 30 minutes from DFW Airport and 15 minutes from Love Field Airport. Please bring any pertinent medical records, x-rays or scans for Dr. Cooper to review at the time of your visit.
Office Visit
It will greatly facilitate your office visit if your medical history and billing paperwork are completed ahead of time. If you are a new patient to The Carrell Clinic, please complete the new patient packet form and bring it with you to your appointment. Please arrive at the office 30 minutes prior to your scheduled appointment. For parking at our clinic, self parking, as well as valet parking are available for your convenience. If you wish, prescriptions can be filled at the Pharmacy at Carrell Clinic Center located on the first floor for your convenience. Please realize that our office practice is very busy. We do the best we can to be punctual, but it is impossible to anticipate the complexity of each patient’s problem on the front end. Therefore, each allotted appointment time is an estimate of the average amount of time that it takes. Please be considerate of this, and realize we are working as hard as we can to remain punctual in our office practice.
Your initial encounter will be with Dr. Cooper’s Orthopedic Physician’s Assistant, Santana Gonzales. He will interview you, do a brief physical exam in order to narrow down the focus of the problem, and then order appropriate x-rays so that Dr. Cooper will have these available at the time of his evaluation of your problem. If MRIs or CT scans are determined to be needed for diagnosis of your problem, these can be scheduled for you (sometimes the same day-subject to availability) at Imaging Center Of North Texas on the 2nd floor of our building for your convenience. Certain minor procedures, such as dressing changes, suture removal, and certain injections, may be performed by our nursing staff or an experienced Physician’s Assistant. This service is available to prevent an unnecessary delay in obtaining an appointment to see Dr. Cooper.

Insurance
For any questions or concerns relating to your insurance plan or current claims, please contact FTGU, at (972) 573-5523 Ext. 185 (Local) or (877) 331-9162 Ext. 185 (Toll-Free).
It is your responsibility to check your insurance benefits and to confirm whether Dr. Cooper is a participating or non-participating provider for your plan. We deal with thousands of carriers and policies and request that the patient confirm this. If Dr. Cooper is not in your network, you can still seek his consultation and/or surgical care. You might be subject to a higher payment obligation according to the terms of your insurance contract. If this creates a financial hardship for you, then you can submit a request for special consideration of your financial obligation. This should be mailed or faxed to our office.
If you are bringing your child in for a school related athletic injury, your school system may have an insurance policy on your child to help cover your claim. Please call your school system to check on any coverage you may have and please bring this information to your appointment, as well as, any secondary insurance you may have on your child.
